This is one of the shows that Nine Inch Nails recorded for the Tension Blu-Ray release (along with Los Angeles 2013 and Houston 2014). This was taken from the Apple Music page of NIN. Ripped in 720p thanks to ninjaw. This is the only footage released from this show.

Was holding out upping this to the archive because I was hoping that this would make a full official release some day. The Tension 2013 Tour was recoreded in LA (along with Vienna and Houston 2014) and I assume was going to make a trilogy of concerts on blu-ray. Sadly, the project was scrapped. But this is what was presented on YouTube from the Staples Center show. Obviously top notch. Maybe one day the full show will be released (along with the other footage)

I found this on YouTube and this is a VHS tape that was made by Trent Reznor for a fan named Michelle in St. Louis. She had written them a bunch of times and missed out on a show in Chicago. So Trent personally made a video for her to watch their performance from 04-11-1990 at the Ritz in New York City! NIN filmed this from the balcony, center. It's good footage with the camera's audio. He also included in the video the long music video of Down In It. Pretty cool to see that he made this for a fan. I have tried contacting nicolasflyer (to where this video came from) on YouTube, but I have never heard back. I would love to have this video from the raw source. But at the moment, this will be the best version that is available. To my knowledge, I have not seen it prior to it being uploaded to YouTube.
